Connie H. Carr, MSN, FNP

Connie Carr provides both clinical and non-clinical research support to the Center for Research at the Institute for Palliative Medicine.

As a Master Facilitator for the Long Term Care Education Project, Connie educates and mentors SDHIPM staff as well as the staff in community-based LTC facilities. The goal of this project is to improve the experience of long term care residents who are approaching the end of their lives through bedside mentoring of the care providers in LTC facilities.

Connie is also the project manager for PAL-MED CONNECT, a 24-hour palliative medicine hotline for healthcare professionals in California, which launched in 2009. PAL-MED CONNECT provides expert advice and educational materials regarding challenging symptoms. Connie coordinates marketing, budgeting, training, and scheduling of the hotline operators.

Connie received her Family Nurse Practitioner certificate from the University of California, San Diego in conjunction with her Master of Science Degree in Nursing from San Diego State University. She also holds a BA in Administration from the University of Nevada, Las Vegas. Connie served in the US Army, both active and reserve duty, for over twenty years. She has worked as a Nurse Practitioner in San Diego County since 1997.
