Connie Leslie, LCSW, ACHP-SW

Connie Leslie is a Licensed Clinical Social Worker and Certified Advanced Hospice and Palliative Care Social Worker. She recently returned from a 5 month sabbatical with Pallium-India where she provided consultation for the development of social work in palliative care. She has been involved in various educational projects for medical, nursing and social work students at San Diego Hospice & Palliative Care. She developed comprehensive training materials for social work practice competencies and preceptor education and has lectured on various topics such as Advanced Directives, Understanding Social Work, Suicide Assessment and Family Meetings.  She provides advanced licensing supervision and continues to deliver direct clinical services to patients and families.

She has practiced in the profession for over 30 years as a clinician, educator and administrator with specialties in mental health and family mediation. She received her Master of Social Work degree from the University of Michigan and is a Board Certified Diplomate in Clinical Social Work.  She has served as an Adjunct Professor and field training instructor at various social work and medical schools in Michigan, Maryland, Virginia, California and the District of Columbia. She serves on the California Board of Behavioral Sciences to provide expert review on social work practice standards.
